Retiring the Ear-to-Ear Scar: The Keyhole Breakthrough at UAB
Until the early 1990s, plastic surgeons elevating an aging brow accepted a brutal trade-off. The standard coronal browlift required a massive ear-to-ear incision across the top of the skull to peel back the scalp and remove a strip of skin. Patients routinely suffered permanent scalp numbness, hair loss along the scar line, and a distorted hairline. Traditionalists insisted that structural elevation was impossible without cutting open the entire scalp, but two surgeons at the University of Alabama at Birmingham refused to accept that premise.
Building on UAB’s rich history of endoscopic innovation, Dr. Luis O. Vasconez and Dr. Grady B. Core asked why a surgeon should strip the entire scalp when the anatomical release could be accomplished through a keyhole. Between 1992 and 1994, Core and Vasconez published landmark clinical findings proving that forehead rejuvenation required only five tiny hairline incisions. Guided by an endoscope passed beneath the scalp, they precisely released the periosteum at the arcus marginalis, eliminating the coronal scar and dramatically reducing patient recovery times.
How Dr. Vasconez Sparked a Global Movement
When Dr. Luis Vasconez arrived as Chief of Plastic Surgery at UAB after leading the program at UCSF, he brought a philosophy centered on open mentorship rather than institutional secrecy. Instead of hoarding their endoscopic techniques, Vasconez used UAB as an international incubator to train visiting surgeons and residents. He recognized that minimally invasive facial surgery would only survive if it became a borderless clinical standard shared across continents.
That open-door philosophy spawned an international network of aesthetic leaders. Trainees like Dr. Renato Saltz applied these endoscopic principles to deep plane facial rejuvenation, Dr. Nazim Çerkeş established structural precision in Istanbul, and Dr. Jorge de la Torre carried the academic torch forward at UAB. By empowering clinicians to refine and expand keyhole techniques worldwide, Vasconez ensured that endoscopic facial surgery evolved from a regional experiment into a global standard of care.

Over three decades, they returned annually to Ambato, performing over seventeen hundred free pediatric surgeries while training local teams.
